Advanced Case Manager

We are seeking a dedicated and compassionate individual to join our team as an Advanced Case Manager. In this role, you will provide high-level support to individuals with complex needs, empowering them to achieve recovery and stability in their lives.
About the Role:
This is a unique opportunity to make a meaningful difference in the lives of vulnerable populations. As an Advanced Case Manager, you will work collaboratively with community services to develop and implement tailored plans that meet the diverse needs of clients.
Key Responsibilities:
- Provide advanced case management support to individuals with complex needs, including mental illness, cognitive impairment, alcohol and substance issues, homelessness, family violence, and limited social supports.
- Work collaboratively with community services to achieve recovery and stabilisation of conditions.
- Develop and implement individualised plans to meet client needs.
- Maintain accurate records and reports.
Requirements:
To be successful in this role, you will have:
- Experience working in a community services or health-related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a small team.
- Commitment to delivering high-quality services to vulnerable populations.
