Job Title

We are seeking a compassionate and skilled Case Manager to join our team in supporting men experiencing homelessness and complex life transitions.

About the Role

The successful candidate will be responsible for developing and implementing strengths-based case plans, providing holistic support and advocating for clients across various systems.

Key Responsibilities:
  • Develop and implement strengths-based case plans in partnership with clients.
  • Support transitions to long-term housing through warm referrals and coordinated outreach.
  • Advocate across housing, health, legal, and community systems to meet client needs.
  • Collaborate with internal teams and external partners to connect clients with relevant services.
  • Maintain accurate records and contribute to service development and continuous improvement.
Requirements

To be successful in this role, you will require a degree qualification in Social Work, Psychology or a related field, along with demonstrated experience in case management within a homelessness or community services setting.
