Focal Community Services is a leading provider of disability support services with a strong commitment to delivering high-quality care and support to individuals with disabilities and their families.

We are seeking an experienced Client Coordinator to join our team in Ipswich. This role will be responsible for coordinating client services, ensuring effective communication between clients, support workers, and management.

Key Responsibilities:

  • To coordinate client services, including intake processes, client assessments, and service planning;
  • To develop and maintain client profiles, including accurate and up-to-date information on client needs and goals;
  • To facilitate communication between clients, support workers, and management, ensuring that all parties are aware of client needs and expectations;
  • To provide support and guidance to support workers, including training and coaching on client-specific needs and requirements;
  • To work collaboratively with the People and Culture team to ensure that support workers are briefed on client-specific health and welfare requirements;
  • To participate in the development and implementation of policies and procedures related to client services;
  • To contribute to the ongoing evaluation and improvement of client services, including identifying areas for improvement and developing solutions.

About You

We are looking for a highly motivated and organized individual with a passion for delivering high-quality client services. The successful candidate will have:

  • A minimum of 2 years' experience in client coordination or a related field;
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients, support workers, and management;
  • Strong administrative skills, including proficiency in digital tools and software;
  • A valid Australian Driver's Licence and comprehensively insured vehicle;
  • Current First Aid & CPR Certificate; and
  • Blue Card & Yellow/NDIS Worker Screening Clearance Card.

Salary Package

The salary for this position is $63,000 per annum, plus generous salary packaging benefits up to $15,900. We also offer learning and development opportunities, access to our Employee Assistance Program, and a supportive People and Culture team.
