Job Title: Therapeutic Case Manager

CatholicCare is seeking a skilled and compassionate Therapeutic Case Manager to join their team. As a key member of the Intensive Therapeutic Care (ITC) program on the Central Coast, you will work closely with a diverse range of clients to provide high-quality case management services.

The successful candidate will be responsible for developing and implementing holistic case plans that promote and support the safety, well-being, and permanency needs of young people in care. This will involve working closely with young people, their families, and stakeholders to identify and address complex trauma and develop pathways for transition through care and ultimately leave care.

Main Responsibilities:
  • Develop and implement holistic case plans that promote and support the safety, well-being, and permanency needs of young people in care.
  • Work closely with young people, their families, and stakeholders to identify and address complex trauma and develop pathways for transition through care and ultimately leave care.
  • Provide timely and effective case management services, including case notes, panel reports, and case work reports.
  • Develop positive relationships with young people, their families, and stakeholders to ensure effective collaboration and outcomes.
  • Make permanency referrals for after care, leaving care, and TSIL/SIL programs.
Requirements:
  • Degree qualifications in Social Work, Psychology, or equivalent, and additional training or qualifications in developmental trauma, child protection, client intervention, and case management.
  • Experience working with children and young people who have experienced complex trauma, and ability to demonstrate case work experience.
  • Ability to work with families who have been in contact with the child protection system.
  • Understanding of the Intensive Therapeutic Care model and its application in practice.
  • Unrestricted driver's license and Working With Children Check and Criminal History Check.
