Family Support Coordinator

We are seeking a highly skilled and compassionate Family Support Coordinator to join our Service Delivery team at Redkite. As an Intake and Connection Officer, you will play a vital role in ensuring families receive timely access to the support they need during their cancer journey.
Key Responsibilities- Initial Contact and Assessment: Act as the first point of contact for incoming referrals, providing navigation for services both internal and external to Redkite depending on the family's needs.
- Client Registration and Information Collection: Facilitate registration and collection of client information as required and within privacy policy.
- Service Promotion and Education: Promote and educate referring organisations and clients on Redkite Services and eligibility requirements.
- Referral Facilitation: Facilitate referrals to the social work team when required.
- Intake Roster Coverage and Support: Cover intake rosters and provide brief emotional support, referral pathways & information to clients, health professionals and other stakeholders.
- Qualifications and Experience: Minimum Certificate IV in Community Services or equivalent, and relevant experience in a community and/or healthcare environment.
- Relationship Building: Demonstrated experience in establishing and maintaining relationships with both internal and external partners, clients and key stakeholders.
- Client Support: Experience in delivering support to clients via phone or email.
- Organisational Skills: Sound organisational skills with proven ability to work autonomously as well as effectively as a member of a high performing team.
- Workload Management: The ability to plan and prioritise workload to ensure that deadlines are met.
- Technical Skills: Strong experience using CRM and Microsoft Office.
