Vulnerable Populations Advocate

We are seeking an experienced and compassionate case manager to join our team in supporting veterans experiencing homelessness or at risk of becoming homeless.
This role involves collaborating closely with the Housing Program Manager and other peer positions to provide critical case management support, empowering veterans to break the cycle of homelessness.
The successful candidate will work closely with veterans to create a Transitional Housing Support Plan with clear objectives and goals, focusing on long-term housing solutions.
Key Responsibilities- Provide primary point of contact for intake and referrals
- Develop and implement Support Plans based on SMART goals
- Support veterans in transitioning back into the community through outreach and follow-up case management
- Manage a diverse caseload effectively, handling difficult situations under pressure
Additional responsibilities may include working occasional weekends or evenings and attending work-related events.
Requirements- Minimum bachelor's degree in health, welfare, community services, or a mental health discipline
- Minimum 5 years demonstrated experience in specialist and complex case management- ideally within mental health, veteran support, homelessness or the community services sectors
- Knowledge of the Australian Defence, ESO, and Veteran communities, with the ability to navigate relevant agencies and departments
- Experience in supporting individuals within the homelessness sector and experience in finding housing solutions
