About the Role

Makom is seeking a dedicated and compassionate Support Professional to join our team. As a Support Professional, you will play a vital role in supporting individuals with Intellectual and Developmental Disabilities (IDD) to lead self-determined and self-directed lives.

Key Responsibilities
  • Assist individuals with IDD in identifying their wants and needs, and develop and maintain skills to facilitate their full potential.
  • Participate in the development of and support individuals in accordance with their Person-Centered Plan (PCP) or Individual Supports Plan (ISP).
  • Provide individualized supports and coaching with Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) as required.
  • Ensure safe and clean living and/or working environments, and report any maintenance needs or assist individuals to obtain assistance from their property management as needed.
  • Be familiar with and comply with medical assessments, dietary restrictions/allergies, care plans, and recommendations provided by medical professionals.
Requirements
  • A minimum of a high school degree; further education/associate degree in human services related field is preferred.
  • Ability to complete required training within the allowed time period.
  • Ability to lift up to 100 pounds with assistance preferred and be capable of kneeling on the floor to help individuals they support.
  • Ability to solve problems, think critically, and provide excellent customer service.
  • Effective communication skills with individuals with IDD, Makom leadership, peers, families, advocates, and external constituencies.
  • Proficiency with Microsoft applications and computer skills.
  • Ability to work independently and with others, exercising professional judgment.
  • Exceptional communication and interpersonal skills.
  • Maintain a professional demeanor, communication, and relationships with people with whom they work.
  • A valid driver's license; ability to pass drug screenings, background checks, reference checks, and other required assessments.
