Client Advocate Lead

About the Role
As a Case Manager, you will be responsible for leading compassionate care across Melbourne's West, Geelong, and Ballarat. This is an ongoing full-time position based in Point Cook or Bacchus Marsh.
This role focuses on delivering high-quality, person-centred services to clients and carers across designated regions. The Case Manager will ensure all services meet funding body and organisational guidelines, applying consumer-directed care principles from initial enquiry through to ongoing support.
Key responsibilities include:
- Conduct comprehensive assessments and develop tailored care plans
- Provide advice on service options and coordinate referrals
- Empower clients and carers to manage their care plans with confidence
- Maintain accurate client records and ensure compliance with funding and legislative requirements
- Monitor service delivery and respond to changing client needs
- Ensure compliance with the Aged Care Quality Standards and relevant legislation
To be successful in this role, you will require:
- Tertiary qualification in Health or a related field (nursing or allied health)
- Experience in case management and care planning
- Strong understanding of aged care reforms and consumer-directed care
- Excellent communication, time management, and networking skills
- Hold a current Victorian driver's licence
- Be open to travel across Western Victoria, Geelong, and Ballarat
- Completion of a National Police Check & up to date Working with Children's Check
