The TCP Coordinator plays a crucial role in improving health outcomes for clients by coordinating safe, responsive, and cost-effective care services. This involves working closely with the Regional Manager to grow our business throughout Victoria and the aged care sector.

The Coordinator will provide initial establishment and review of the TCP care plan following a comprehensive standardized assessment, ensuring alignment with national Transitional Care Program guidelines and the hospital TCP team.

Key responsibilities include coordination of direct client care, highlighting the need for referral to multidisciplinary or specialty services, and assisting clients to set realistic healthcare goals.

Required Skills and Qualifications:
  • Current AHPRA registration as a Registered Nurse (no conditions)
  • High level of communication skills displaying a commitment to care and empathy for clients and staff
  • Strong organizational and time management skills
  • Willingness to undertake or holds a current police check and working with children's check
  • Mandatory current driver's license
