Community Health Nurse Specialist

We are seeking a highly skilled and experienced Community Health Nurse Specialist to join our team at Primary Care Connect.
About the RoleThe Community Health Nurse Specialist will be responsible for providing clinical assessment and support services for individuals undertaking withdrawal from alcohol and other drugs (AOD) in a non-residential setting.
This role involves working with clients who have low to medium-risk dependencies, as well as those with higher-risk requirements, including targeted step-down withdrawal support following residential treatment.
The successful candidate will work within an integrated and coordinated care pathway, linking clients to relevant support services, including AOD counselling and care and recovery.
Key responsibilities include:
- Clinical assessment and risk assessment of clients
- Development of individualized care plans and interventions
- Provision of information and support for clients undertaking withdrawal
- Liaison with external organizations and services
To be successful in this role, you will need:
- Current registration as a Registered Nurse Division 1 with Australian Health Practitioner Registration Australia (AHPRA)
- Extensive experience working with AOD clients
- Minimum core competency qualification (AOD Skill Set) or completion within six months of commencing employment
- Demonstrated proficiency in supporting AOD withdrawal services, including clinical assessment, care planning, and interventions
- Knowledge of clinical guidelines for supporting clients undertaking AOD withdrawal
