Job Description

The Chronic Disease Monitoring Service is a key member of our Community Health Leadership Team. As a Team Leader, you will manage a team of Registered Nurses and Allied Health staff, with Medical support. Your primary goal will be to improve chronic disease management and prevent hospital admissions by implementing a new model of care.

Key Responsibilities

  1. Manage a team of Registered Nurses and Allied Health staff, providing line management, supervision, support, and coaching.
  2. Collaborate with key stakeholders to implement the new model of care, considering clinical risk, inclusion criteria, intake processes, and clinical escalation processes.
  3. Work in partnership with other clinicians and managers to ensure the model is integrated with other services and streamline the journey for patients.

Required Skills and Qualifications

  • Registered Nurse (APHRA registered)
  • Post graduate cert/diploma in Critical Care or similar
  • Experience and advanced skills in clinical heart failure/chronic disease management
  • Demonstrated leadership experience, with the ability to work autonomously
  • A Clear and Confident Communicator, both written and verbally
