Senior Chronic Disease Management Professional

Job Role:
The Chronic Disease Manager will be instrumental in assisting healthcare professionals and patients to identify and manage chronic conditions, thereby reducing hospital admissions.
Key Responsibilities:
- Create personalized care strategies to empower patients to effectively manage their health
- Conduct comprehensive health assessments to facilitate early detection of chronic diseases
- Collaborate with healthcare teams to promote preventive care practices
- Regularly review patient care plans to ensure optimal health outcomes
- Manage administrative tasks related to CDM appointments
Essential Requirements:
- AHPRA registration as an Enrolled or Registered Nurse
- Demonstrated experience in Chronic Disease Management and health assessments
- Strong organizational and time management skills
- Excellent communication and customer service skills
Qualifications:
To be successful in this role, applicants must possess a strong foundation in nursing principles and have the ability to work independently as well as collaboratively within a healthcare team.
