Job Overview:
The Chronic Disease Management (CDM) Nurse plays a pivotal role in preventing hospitalizations and enhancing patient health outcomes.
Key Responsibilities:
- Develop and implement evidence-based care plans to manage chronic diseases effectively
- Empower patients to take ownership of their healthcare through education and support
- Counsel patients on healthy behaviors and disease self-management strategies
- Collaborate with General Practitioners to promote preventive healthcare services
- Conduct comprehensive health assessments to identify and manage chronic diseases
- Regularly review and update patient care plans to ensure optimal health outcomes
- Manage administrative tasks related to CDM appointments efficiently
Essential Qualifications:
- AHPRA registration as an Enrolled or Registered Nurse
- Experience in Chronic Disease Management and health assessment practices
- Knowledge of Health Assessment is highly desirable
- Excellent organizational and time management skills
- Strong communication and customer service skills
