Job Description

This role focuses on supporting patients with chronic conditions to live well at home. You will be responsible for conducting comprehensive health assessments, developing care plans, and coordinating referrals to ensure each person receives the care they need to thrive.

Your key responsibilities will include:

  • Supporting patients to set goals and develop personalized care plans
  • Initiating referrals to healthcare professionals and services as needed
  • Participating in clinical supervision and stakeholder engagement to ensure high-quality, customer-focused care
