Key Responsibilities:- Develop and implement comprehensive care plans and coordinated team care arrangements for patients with chronic diseases.
- Empower patients to take ownership of their health by educating them on behavioral change principles and facilitating access to ongoing care from a multidisciplinary team.
- Conduct thorough health assessments to identify early signs of chronic disease and collaborate with healthcare professionals to promote preventative care throughout the practice.
- Manage administrative tasks related to CDM appointments and ensure regular review of patient care plans.
- AHPRA Registered Nurse preferred.
- Proven experience in Chronic Disease Management and health assessments.
- Strong organizational and time management skills, as well as excellent communication and customer service skills.
