You will contribute to reducing avoidable hospital admissions, building the capacity of services, and supporting individuals to confidently manage their health through integrated care coordination.
- Conduct comprehensive health assessments and identify priority issues
- Support patients to set goals and develop care plans
- Initiate referrals to health professionals and services
- Participate in clinical supervision and stakeholder engagement
- Ensure services are high-quality, customer-focused, and evidence-based
- Maintain accurate records and uphold professional standards
