Chronic Disease Care Role
We are seeking a skilled professional to support patients with chronic diseases in accessing necessary services according to their General Practitioner Care Plan.
The successful candidate will provide clinical care, arrange services and assist patients in regular reviews with their primary care providers.
Key Responsibilities
- Supporting patients throughout their care pathway, from encouragement to accessing multidisciplinary care.
- Providing clinical care and arranging services as needed.
- Assisting patients in regular reviews with their primary care providers.
Requirements
- Degree in Nursing or Allied Health profession.
- Experience in chronic disease management and care coordination.
- Strong communication and interpersonal skills.
