We are seeking a dedicated professional to join our Homelessness Stream as an Outreach Case Manager.

About This Role

This is a key position within our team, focusing on providing holistic, client-centered case management services to individuals and families experiencing homelessness. You will work collaboratively with internal and external stakeholders to deliver effective outreach engagement and service delivery, ensuring timely and consistent outcomes.

Your primary objective will be to support clients in achieving housing stability, social inclusion, and stronger connections with community supports and services. You will incorporate care planning for dependants based on individual circumstances, either separately or within the caregiver's plan.

The successful candidate will have tertiary qualifications in Community Services, Social Work, or a related field, and previous experience in the homelessness sector. A strong understanding of the challenges faced by individuals and families experiencing homelessness is essential.

Key Responsibilities:

  • Deliver comprehensive intake and assessments using a trauma-informed and Housing First approach.
  • Develop and regularly review individualized case plans tailored to each participant's goals, background, and needs.
  • Cultivate strong working relationships with internal and external stakeholders, including active participation in interagency meetings.
