Job Title: Homeless Support Services Coordinator
About the Role:The Case Manager will provide high-quality case management services to clients at risk of homelessness. The role involves developing individualized plans, guiding clients in achieving realistic goals, and maximizing their opportunities for stable housing.
Key Responsibilities:- Develop comprehensive case plans that address client needs and goals.
- Guide clients to set achievable objectives that align with their strengths and challenges.
- Maintain accurate and timely records of client progress and outcomes.
- R elevant tertiary qualifications (e.g., Diploma of Community Services Work, Bachelor of Community Welfare).
- Minimum 4 years of experience in case management, preferably in the CALD community.
- Demonstrated ability to work effectively with women experiencing mental health issues, substance abuse, and domestic violence.
This is a full-time position working 9am-4:30pm, Monday to Thursday. A 4-day workweek may be considered. No on-call responsibilities.
