The Assertive Outreach Drug and Alcohol Clinician position at St Vincent's Health Australia is a critical role that requires a highly skilled and compassionate professional to provide evidence-based specialist healthcare, support, and care coordination to individuals experiencing complex unmet health and support needs.
Key Responsibilities:
- Provide specialist assessment, treatment, referral, and care coordination services to individuals experiencing substance use and addiction, as well as comorbid health and support needs.
- Collaborate with clients, multidisciplinary teams, and key stakeholders to address clients' cultural, holistic health, and support needs.
- Utilize principles of trauma-informed care and strengths-based, harm reduction best practice approaches to address clients' needs.
- Develop and implement individualized care plans that address clients' complex health and support needs.
- Work collaboratively with community-based services and organizations to provide comprehensive support and care to clients.
Requirements:
- Post-qualification experience in a related health discipline, with a minimum of 3 years of experience in providing community-based specialist assessment, treatment, and care coordination services.
- Registration with a relevant professional body, if applicable.
- Demonstrated understanding of and commitment to issues relating to homelessness, harm minimization, and trauma-informed care.
- Ability to work collaboratively and communicate effectively with clients, multidisciplinary teams, and key stakeholders.
- Demonstrated commitment to clinical supervision, ongoing professional development, and service improvement.
- Effective computer literacy with email and an electronic patient record system.
