About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team at The Salvation Army Aged Care. As a Nurse Care Coordinator, you will play a vital role in providing clinical supervision and coordinating the delivery of safe, effective, and individualized care to our care recipients and their families.

Key Responsibilities
  • Provide clinical supervision and guidance to care staff to ensure high-quality care is delivered.
  • Coordinate the development and implementation of individualized care plans that meet the unique needs of each care recipient.
  • Assist care recipients to lead an active and meaningful life as independently as possible, in accordance with The Salvation Army's Mission, Vision, and Values.
  • Manage staff and physical resources to ensure efficient and effective care delivery.
  • Maintain accurate and up-to-date clinical records and documentation to ensure legal adequacy and compliance with regulatory requirements.
  • Liaise with doctors and other health professionals to ensure seamless care coordination and delivery.
About You

To be successful in this role, you will possess:

  • A strong understanding of and commitment to The Salvation Army's Mission, Vision, and Values.
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency.
  • Demonstrated aged care experience and the ability to work as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to promote education and leadership in aged care.
  • Understanding of and commitment to continuous quality improvement.
  • Experience in developing care plans and coordinating care delivery.
