About the Role

The Salvation Army is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will be responsible for coordinating and managing a comprehensive range of services and supports for our clients. This will involve building strong relationships with clients and their families, as well as working closely with our team to ensure seamless delivery of care.

Key Responsibilities
  • Conduct initial assessments and onboarding for new clients, and maintain ongoing assessment and relationship-building with clients and their representatives.
  • Develop, implement, and monitor consumer-directed care plans in collaboration with clients.
  • Provide consumer-directed case management services to clients, acting as the main point of contact between clients, their informal carers, Home Care Workers, and external service providers.
  • Establish and maintain positive working relationships with all members of the Salvation Army Home Care team and the broader Aged Care Support Services team.
About You
  • You will have a strong appreciation for and ability to promote, support, and model the Christian values and ethos of The Salvation Army.
  • You will hold tertiary qualifications in a relevant field or have a minimum of three years' industry experience.
  • You will be motivated to work with older people living in the community and have an understanding of consumer-directed care and enablement approaches.
  • You will have proven leadership experience in overseeing, coordinating, and implementing consumer-directed care services within a Home Care setting.
  • You will possess excellent oral and written interpersonal skills, a strong customer service focus, and the ability to negotiate and influence others.
  • You will hold a current driver's license, have a good driving record, and be willing to travel for work.
