
Age Appropriate Support Specialist

We are seeking a skilled and compassionate Support Services Coordinator to join our multidisciplinary team. As a Support Services Coordinator, you will play a vital role in supporting clients to participate in age-appropriate activities and tasks.
Key Responsibilities:
- Support clients to access activities and outings in the community.
- Provide direct assistance to each client in accordance with their service plan and as directed.
- Monitor the safety and well-being of the clients and report any concerns to the Manager.
- Ensure completion of appropriate documentation.
- Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
Required Qualifications and Skills:
- Bachelor's degree in psychology, social work, or related field.
- Membership with AASW
- Current National Police Check
- Working with Children Check
- First Aid certificate and CPR
- NDIS Workers Orientation
Skill Requirements:
- Demonstrated proficient written, verbal and interpersonal communication skills.
- Demonstrated ability to work with minimal supervision, and collaboratively as a member of a multidisciplinary team.
