About this role
We are seeking a skilled and compassionate Dementia Support Professional to join our team. As a key member, you will play a vital role in delivering high-quality dementia support services.
Key Responsibilities
- Provide expert support to families and caregivers, empowering them to understand the impact of dementia on individuals.
- Educate carers and clinicians on effective strategies to prevent and respond to behaviour changes, promoting person-centred care.
- Develop comprehensive reports to inform evidence-based recommendations, driving improved outcomes for individuals living with dementia.
- Carry out holistic assessments of individuals and their care networks, identifying areas for growth and development.
- Collaborate with our multidisciplinary team to build capability across sectors, fostering a culture of excellence in dementia care.
- Contribute to research projects and programs, shaping the future of dementia support.
Requirements
- Experience supporting people living with dementia and a strong commitment to a person-centred approach.
- Relevant qualifications in Allied Health, Nursing, Aged Care, or Mental Health.
- Excellent negotiation, communication, and relationship building skills.
- Ability to prioritise tasks and meet deadlines, ensuring seamless service delivery.
