We are seeking a skilled and compassionate Senior Home Care Specialist to join our team. As a senior member of the home care team, you will play a vital role in developing care plans that align with client needs and aspirations for an independent, safe, and comfortable life in their own home.

About the Role:
  • Conduct comprehensive assessments and develop person-centred care plans focused on fostering client independence, wellness, and reablement
  • Collaborate with clients and their families to understand how they can utilise their funding to achieve positive outcomes that optimise their health, wellbeing, and safety
  • Build effective partnerships with stakeholders, including clinical experts, allied health professionals, and support workers, to ensure our clients receive the right care and support to meet their individual needs
Requirements:
  • A degree in a related field or equivalent experience in the health and/or aged care sector
  • Experience in delivering customer service excellence and working with diverse client groups
  • Strong communication and interpersonal skills, with the ability to build rapport with clients, families, and stakeholders
  • Attention to detail, positive attitude, and strong time management skills
