Key Responsibilities:
  • Support individuals, families, caregivers, and clinical staff to comprehend how dementia impacts each person
  • Equip carers and clinicians with the knowledge and skills necessary to prevent and respond to behaviour changes through modelling, coaching, and development of resources including behaviour support planning
  • Synthesise information through observation and written documentation to provide recommendations that are relationship-centred and informed by evidence-based best practice
  • Develop reports contextualised to individual situations that effect change on the ground and improve quality of life for people living with dementia
  • Undertake assessments to understand the holistic experience of the person and their care network
Requirements:
  • Experience supporting people living with dementia – a person-centred approach to care, with a genuine commitment to supporting individuals and their families
  • Experience working with older people, ideally within Australian residential and/or community aged care settings, including an understanding of the needs of older people in regional and remote areas
  • Ability to share knowledge in a way that supports change at all levels
  • High level of negotiation, communication, and relationship building skills
  • Excellent problem-solving and decision-making skills
  • Ability to prioritise multiple tasks and meet deadlines
