
Senior Client Care Coordinator

We are seeking a skilled professional to deliver exceptional client care and support to older Australians living at home. The successful candidate will be responsible for providing high-quality care planning, assessment, and coordination services.
Key Responsibilities:
- Develop, monitor, and reassess care plans in consultation with clients, families, and healthcare professionals.
- Deliver excellent client care including Care Planning, Assessment, and Coordination.
- Use customer service and aged care management skills to make a positive impact on the community.
Requirements:
- Experience in aged care, with knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards.
- Frontline management skills.
- Vietnamese speaking desirable.
- Strong computer skills in CRM systems, database management, Microsoft Office.
- Current First Aid and CPR certificate.
- The right to work in Australia.
- A current driver's license, registration, and access to your own car with comprehensive insurance.
- National Police Clearance or willingness to obtain (myHomecare will organise).
- Proof of Covid Vaccination.
