
Client Care Coordinator

At the heart of our business is a passion for delivering high-quality care to individuals in need. We are seeking a skilled Client Care Coordinator to lead and coordinate all activities related to client care, ensuring seamless service delivery.
As a key member of our team, you will be responsible for managing the overall client experience, championing their best interests while driving business goals. You will build trusting relationships with clients, their families, and representatives, conducting regular quality assurance visits and collaborating on care plans.

Key Responsibilities:
- Build strong relationships with prospective and existing clients, their families, and representatives
- Conduct regular quality assurance visits (in person or via phone) to monitor care, address changing needs, and recommend additional services as required
- Collaborate with clients, families, caregivers, and other stakeholders to ensure care plans remain aligned with evolving needs and preferences
About You:
- A tertiary qualification in a health-related field (e.g., social work, allied health) with at least 1 year of experience working with Home Care Packages and seniors in aged care
- Strong organisational skills with the ability to manage time effectively, multitask, and adapt to shifting priorities under pressure
- Excellent written and verbal communication skills, with accurate data entry and proficiency in Microsoft Office
