Family Health Advocate Role
About the RoleWe are seeking a skilled Family Health Advocate to join our team. As a Family Health Advocate, you will play a vital role in promoting health and wellbeing for children and families from diverse backgrounds.
Your Key Responsibilities:
- Develop comprehensive health plans for young children, aligning with Department of Health guidelines.
- Provide tailored services and programs to meet the unique needs of families from various cultural and social contexts.
- Offer anticipatory guidance, deliver learning and health information, initial counseling, referrals, and make notifications as required.
- Maintain accurate and confidential records of daily activities and family profiles, adhering to relevant standards and privacy requirements.
- Collaborate with multidisciplinary teams and adhere to Council policy, protocol, and Department of Health guidelines.
About You:
- Registered Nurse and Midwife with Australian Health Practitioner Regulation Agency accreditation, holding additional qualifications in Maternal and Child Health.
- Excellent communication and writing skills, enabling engagement with diverse backgrounds and cultures.
- Demonstrated decision-making skills based on professional knowledge and expertise.
