Immunology Specialist Social Worker

We are seeking a highly motivated Level 3 Immunology Specialist Social Worker to join our clinical team. Our progressive department offers development opportunities for our staff and a caring environment for the delivery of high-quality patient-centered care.

The successful applicant will be responsible for providing a comprehensive social work service to patients/clients and their families, liaison with other healthcare professionals within St Vincent's Hospital Sydney and with external agencies in the area of allocation.

Key Responsibilities:
  • Provide consultancy services, advocacy, clinical assessments and counseling to patients, families, and carers.
  • Contribute to complex transfer of care planning and provide clinical cover for colleagues as required.
  • Provide consultation to team members and education for patients and staff as required.
Requirements:

To be successful in this role, you will have:

  • Personal integrity and demonstrated commitment to the Philosophy, Mission, and Values of Mary Aikenhead Ministries and St Vincent's Health Australia.
  • Bachelor of Social Work from a recognized tertiary provider or equivalent tertiary qualification with eligibility for membership of the Australian Association of Social Workers (AASW).
  • High-level understanding and experience of complex social issues for patients, families, and carers with regard to clinical immunology, infectious diseases, viral hepatitis, and HIV infection, both within an inpatient and outpatient setting.
  • Demonstrated high-level clinical skills and knowledge in complex psychosocial assessment, counseling, therapy interventions, case management, advocacy, and discharge planning with clinical experience in a hospital or health-related field as a practicing Social Worker.
