
Comprehensive Social Care Professional

Role Summary:
This role is responsible for delivering high-quality social work services to patients, families, and carers within a hospital and local health district setting.
The successful applicant will have the opportunity to exercise independent professional judgment on routine matters, with direct professional supervision available when performing novel, complex, or critical tasks.
Key Responsibilities:
- Provide comprehensive assessments and interventions to patients, families, and carers
- Develop and implement individualized care plans in collaboration with multidisciplinary teams
- Participate in quality improvement activities and contribute to policy development
- Demonstrate expertise in clinical practice and stay up-to-date with industry developments
- Collaborate effectively with colleagues and external partners to achieve desired outcomes
Requirements:
- Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW)
- Demonstrated clinical experience or understanding of client needs
- Strong time management and communication skills, including report writing
- Ability to work independently and as part of a team
- Knowledge of quality assurance and clinical practice improvement activities
Benefits:
- Competitive salary range: $70,944 - $101,211 per annum
- Paid Allocated Day Off (ADO) every month, in addition to annual leave
