Job Description

The South Wingecarribee Transition Care Program is seeking a skilled Social Worker to join their dynamic multidisciplinary team in Bowral. This is an exciting opportunity to provide expert social work interventions to clients and their carers, facilitating optimal health outcomes.

As a key member of the TACP team, you will be responsible for assessing client needs, providing appropriate support, and developing discharge plans. Your clinical caseload will involve working closely with clients, carers, and healthcare professionals to ensure seamless care transitions.

Key responsibilities include:

  • Providing goal-oriented social work interventions to enhance client well-being
  • Conducting thorough assessments to identify client needs and develop individualized plans
  • Collaborating with multidisciplinary teams to ensure comprehensive care delivery
Required Skills & Qualifications

To succeed in this role, you will need:

  • A degree in Social Work, qualifying for membership of the Australian Association of Social Workers (AASW)
  • Demonstrated experience in a health-related field, with a focus on social work interventions
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, carers, and healthcare professionals
  • Proven ability to work independently and collaboratively within a fast-paced, dynamic environment
