Job Title: Social Services Professional

We are seeking an experienced and compassionate Social Services Professional to join our dedicated team. As a key member of our organization, you will play a vital role in providing high-quality care to our patients.

About the Role:

  • Provide exceptional social work services addressing the complex interplay between individuals, their environments, and healthcare processes.
  • Deliver comprehensive support including crisis intervention, psychosocial assessments, counseling, advocacy, case management, and therapeutic support.
  • Foster strong relationships with patients and families, identifying psychosocial challenges impacting recovery and transition through the healthcare system.

Your Skills and Qualifications:

  • Degree in Social Work with eligibility for professional registration.
  • Demonstrated understanding of the psycho-social impact of unexpected hospitalization due to illness, accident, or trauma.
  • Ability to complete psychosocial assessments and develop effective discharge and care plans within tight timeframes.
  • Experience as a social worker in acute aged care settings, including applications and hearings related to NCAT/Guardianship Division, elder abuse, and complex family situations.
  • Understanding of the effect of decreasing independence on carers and families.
  • Strong communication and interpersonal skills, with the ability to work collaboratively within a multi-disciplinary team.
