The role of an ACAT Assessor is to provide comprehensive assessments, support and management to the target population in the community, hospitals and aged care facilities.
About the PositionThe primary function of this position is to work within a multidisciplinary team to deliver holistic care to clients from diverse backgrounds.
Key Responsibilities:- To conduct thorough assessments of clients' needs and develop individualised care plans
- To work collaboratively with other healthcare professionals to ensure high-quality care
- To maintain accurate records and reports
To be successful in this role, you will require:
- A degree in a relevant field such as Occupational Therapy, Social Work or Physiotherapy
- Current registration with the relevant professional body
- Minimum 3 years post-graduate experience working with older people in an acute, aged care and/or community setting
- Demonstrated knowledge of Aged Care Systems and experience working with clients from diverse cultural backgrounds
