We are seeking an enthusiastic and skilled Housing Support Worker to join our dynamic team.

This role offers a unique opportunity to empower older Australians who have experienced or are at risk of homelessness, providing vital support services to help them maintain their independence and live safely in their own homes.

As a Housing Support Worker, you will be responsible for case management of referrals from Housing Connect, aligned with the Housing Connect system. You will also link and refer clients to allied and clinical services, social, and recreational activities.

You will facilitate access to Home Care Packages (HCP) and NDIS supports through referrals, service navigation, and case management. Additionally, you will review client files across program areas to ensure accurate, timely documentation and monitor service quality.

Responding to client feedback and complaints is crucial, and you will utilise sector relationships to assist access to other services and systems.

With support from the southern tenancy team, you will undertake tenancy-related tasks.

Requirements:
  • A tertiary qualification in Social Work, Certificate IV in Community Services, Community Housing, or equivalent experience in homelessness or community services (minimum 2 years).
  • At least 2 years' experience working with vulnerable populations, including aged care, mental health, or allied health services.
  • A current driver's license.
  • First Aid Certificate Level 2.
  • Experience as a support worker in homelessness or intensive case management.
  • Understanding of the housing and homelessness sector, My Aged Care, NDIS, and related systems.
  • Knowledge of the Residential Tenancy Act (Tasmania) and ability to undertake tenancy-related tasks.
