The successful applicant will work in partnership with families to empower and build their capacity for effective change, addressing identified family challenges. The role involves significant outreach efforts, engaging families in their homes and fostering ongoing partnerships. Initial engagement translates into a strong partnership, where the practitioner delivers interventions that build family awareness and insight into their history and current challenges. This enables families to self-identify key challenges and lead their own change.
Key Responsibilities- Meet individual case performance targets as per weekly, monthly and quarterly dashboard system.
- Provide high-quality practice in all interactions with clients, consistent with organisational values and vision.
- Maintain accurate and up-to-date record keeping that adheres to operational requirements.
- Engage clients through assertive outreach, promoting positive and warm relationships.
- Facilitate client-led assessments and Care Plans to identify client goals, including entire families where appropriate.
- Deliver targeted interventions and family facilitated group meetings to achieve client goals.
- Ensure high quality management practices through use of LogiQC quality management systems and MobNet tasks.
- Cultivate productive working relationships at all levels, supporting internal and external relationships, cooperation, trust and respect.
- Current Bachelor's Degree in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services or Family Intervention with 2 years' relevant experience in social services field.
OR
- Current Diploma in Community Services, Social Work, Human Services, Child and Family, Allied Health, Youth Work or Child and Family Intervention with 3 years' experience in social services field.
AND
- At least 1 year of abovementioned experience in a case management role.
- Ability to work in a performance-driven environment to meet individual weekly, monthly and quarterly performance targets.
- Capacity to build client agency, self-efficacy and facilitate change empowerment, discouraging client dependence on supports.
- Knowledge and ability to provide behaviour change interventions that build client self-awareness and active change in achieving their identified goals.
- Ability to work with the whole-of-family, irrespective of primary client, meeting the totality of family needs.
- Capacity to coordinate multiple services providing access to holistic care for clients, integrating internal agency supports across multiple teams.
- Understanding of factors contributing to vulnerability, disengagement and intergenerational trauma of client group.
- Experience with case management for high-risk and/or crisis clients, with high-level planning and organisation skills.
- Knowledge of available supports targeting clients in mental health, substance misuse, education, employment and housing areas.
- Requirement to travel across Logan and/or Brisbane region, as per program location.
- Current drivers' licence.
- Valid police check.
- Flexibility to work outside core business hours to meet individual client needs and provide responsive care.
