The role of a Family Wellbeing Service Practitioner is to empower families to overcome their identified challenges through outreach-based partnerships. The practitioner works closely with families in their homes, building trust and facilitating strong ongoing relationships.
This position requires the ability to engage clients effectively, provide high-quality practice aligned with organizational values, and facilitate client-led assessment and Care Plans to identify goals. The practitioner will also provide targeted interventions and family-facilitated group meetings to achieve client goals.
Key Responsibilities:- Meet performance targets as per dashboards.
- Provide quality practice in all interactions with clients.
- Engage clients through a positive outreach approach.
- Facilitate client-led assessment and Care Plan to identify goals.
- Provide targeted interventions and family-facilitated group meetings.
- Bachelor's Degree in Social Work or related field with 2 years of experience in social services.
- Diploma in Community Services or related field with 3 years of experience in social services.
- At least 1 year of experience in case management role.
