We are seeking a highly skilled Home and Family Coordinator to join our team. The successful candidate will work closely with individuals and families in partnership to empower them to overcome their identified housing and wellbeing needs.
This role involves working in an outreach-based setting, engaging with clients in crisis accommodation, street outreach, and refuges, and translating initial engagement into a strong ongoing partnership. You will provide high-quality support and interventions based on the program model of care, and ensure that all interactions with clients are consistent with our values and vision.
The ideal candidate will have a passion for working with vulnerable populations and possess excellent communication and interpersonal skills. You will be required to work collaboratively with colleagues to provide holistic care to clients, particularly integrating agency supports across multiple teams.
Key Responsibilities:
- Provide high-quality case management practices and record-keeping that adhere to operational requirements.
- Facilitate client-led assessments and care plans to identify client goals, including whole family involvement when appropriate.
- Provide role-specific targeted interventions and family group meetings to achieve client goals.
- Work collaboratively with colleagues to provide holistic care to clients, including integrating agency supports across multiple teams.
Requirements:
- Current Cert IV in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Child and Family Intervention or related field, with at least 12 months experience in a case management role.
- OR Current graduate university bachelor's degree in social work, human services, social science, health, allied health, youth work, community services, criminology, with at least a 10-week full-time placement in a social service field.
Benefits:
- Starting salary of $82,732 per annum (dependent on qualifications and experience) + 12% superannuation.
