The role of the Family Wellbeing Service Practitioner is to empower families by working with them and building their capacity, enabling them to make effective changes and overcome challenges.
This involves outreach-based work, engaging families in their homes and building a strong partnership. The goal is to deliver interventions that promote family awareness and insight into their history and current challenges.
Key Responsibilities- Meet performance targets as per weekly, monthly, and quarterly dashboards.
- Provide high-quality practice aligned with organisational values and vision.
- Deliver targeted interventions and group meetings to achieve client goals.
- A current Bachelor's Degree in Social Work or a related field, with 2 years of experience in social services.
- Or: A current Diploma in Community Services, with 3 years of experience and 1 year in a case management role.
- Ability to work in a performance-driven environment and meet targets.
- Knowledge of behaviour change interventions and factors contributing to vulnerability and intergenerational trauma.
- Willingness to travel across the region as needed.
- A valid driver's licence and a current Police Check.
- Flexibility to work outside core business hours.
