About the Role

We are seeking a dedicated and compassionate Case Manager to join our team at The Salvation Army. As a Case Manager, you will play a vital role in supporting young people who are homeless or at risk of homelessness.

Key Responsibilities:

  • Develop and implement individualized case management plans that address the unique needs and goals of each young person.
  • Provide support and guidance to young people to enhance their skills and build positive connections within the community.
  • Collaborate with other organizations and agencies to ensure a comprehensive and coordinated approach to supporting young people.
  • Maintain accurate and up-to-date records and documentation of case management activities and outcomes.
  • Identify and respond to safety and well-being concerns, and follow established protocols for reporting and addressing these issues.
Requirements

To be successful in this role, you will need:

  • A minimum of a Diploma-level qualification in a relevant field, such as Youth Work or Community Services.
  • Experience working with young people who are homeless or at risk of homelessness.
  • Strong communication and interpersonal skills, with the ability to build rapport with young people and work collaboratively with other professionals.
  • A current ACT Driver's License and a current Senior First Aid Certification.
  • ACT Working with Vulnerable People Certification.
