About the Role

We are seeking a dedicated Family Support Case Manager to join our team in Dandenong. In this role, you will provide essential support to families referred through various services, helping them navigate and access the resources they need to thrive.

Key Responsibilities
  • Case Management:
    • Assess and manage cases of families referred from The Orange Door and Child Protection.
    • Maintain detailed records of assessments, interactions, and case progress.
    • Work with families to identify their needs and connect them with appropriate services.
    • Develop and implement intervention plans, setting clear goals and timelines.
    • Address health, development, and family-related issues, referring to relevant services as needed.
  • Support and Advocacy:
    • Offer guidance on parental health, family planning, and family violence.
    • Enhance families' access to community resources and support groups.
    • Advocate on behalf of families with other services, agencies, and government departments.
    • Collaborate with a multi-disciplinary team to provide culturally sensitive and holistic care.
  • Program Development:
    • Create and evaluate group programs for parents and children.
    • Ensure families receive comprehensive information about available services and use interpreters if necessary.
  • Compliance and Reporting:
    • Adhere to legal and policy guidelines, including Child Protection Protocols and Mandatory Reporting.
    • Maintain confidentiality and comply with privacy laws.
    • Stay updated with relevant legislation and organizational strategies.
  • Community Engagement:
    • Build strong connections with community and government agencies.
    • Improve referral pathways and support systems for vulnerable families.
  • Continuous Improvement:
    • Engage in service planning and use feedback to enhance delivery.
    • Participate in quality assurance and professional development activities.
    Requirements
    • Educational Background: Degree in Social Work, Psychology, Behavioral Sciences, or a related field.
    • Experience: Relevant experience in working with vulnerable families
    • Communication: Strong interpersonal and communication skills.
    • Organizational Skills: Ability to manage time effectively and work cooperatively with various organizations.
    • Recent National Police Check (issued within the last 6 months)
    • Working with Children Check
    • valid Victoria Full Driving Licence.
