Coordinated Service Delivery Specialist

We are seeking a professional with exceptional coordination skills to provide service delivery for our clients.

This role involves:

  • Directly coordinating service delivery for clients
  • Working closely with other coordinators and HR to coordinate staffing in response to client needs
  • Managing and supervising support staff delivering services to clients

The ideal candidate will have experience working with complex clients, including those with behavioural issues or previous experience with Positive Behaviour Support Plans.

Key requirements include knowledge of NDIS Practice Standards, experience in implementing these standards, and managing client NDIS plans/budgets.

The successful candidate will be required to have a comprehensive understanding of documentation, including recording progress notes, incidents, and new client intake assessments.

Strong communication and interpersonal skills are essential, with the ability to work effectively with families with complex and multiple needs.

Mandatory Requirements
  • Accredited Social Worker/Accredited Mental Health Social Worker registration with AASW
  • Yellow Card or NDIS worker screening clearance
  • Blue Card
  • Current First Aid and CPR (or immediately obtain)
  • Driver's License and comprehensive vehicle insurance and vehicle for work use
  • Positive attitude and passion to improving the lives of people
  • Understanding NDIS Principles
  • Covid Vaccination Certificate
