**CARE COORDINATOR**

- **Location: Brisbane and surrounds**
- **Hours: Part-time 15-20 hours a week**

**TAUS Care is a multicultural in-home and community support service provider based in Brisbane, Queensland. It has a specific focus on supporting participants of the National Disability Insurance Scheme (NDIS) with the intention to expand into other sectors such as Aged Care in the future.**

**We aim to deliver innovative and effective care solutions and services to our Queensland community. Our diverse range of services enables us to provide end-to-end, holistic care and support as participants' needs change.**

**Job description**
- **Directly coordinate service delivery for TAUS Care clients.**:

- **Work closely with other Care Coordinators and HR to coordinate staffing in response to client needs.**:

- **Manage and supervise support staff delivering services to TAUS Care clients.**:

- **Experience working with complex clients including clients with behavioural issues or previous experience with clients under Positive Behaviour Support Plans.**:

- **Working with dual mental health disorder clients.**:

- **Ability to work with families with complex and multiple needs.**:

- **Working from a client centred perspective where the clients and carers are experts in their own lives.**:

- **Willing to travel within Brisbane and the greater Brisbane region when required**:

- **Knowledge of and experience in implementing NDIS Practice Standards & managing client NDIS plan/budget.**:

- **Competency in use of Microsoft Office Suite**:

- **Electronic reporting of Incidents using NDIS Quality Safeguards Commission portal.**:

- **Comprehensive knowledge and experience in documentation of records / progress notes / incidents.**:

- **New client intake assessments**:

- **Experience in managing the plan review process and preparing regular participant progress report for management.**

**Highly Desirable**

**Accredited Social Worker/Accredited Mental Health Social Worker registration with AASW.**

**Mandatory Requirements**
- **Yellow Card or NDIS worker screening clearance.**:

- **Blue Card.**:

- **Current First Aid and CPR (or immediately obtain).**:

- **Driver's License and comprehensive vehicle insurance and vehicle for work use**:

- **Positive attitude and passion to improving the lives of people.**:

- **Willingness to adhere to the NDIS Code of Conduct Summary for workers**:

- **Understanding NDIS Principles**:

- **Managing client budget and supports with mínimal supervision.**:

- **COVID Vaccination Certificate**
