Job Summary

We are seeking an experienced SDA Journey Manager to join our team. As an SDA Journey Manager, you will play a critical role in supporting individuals with disabilities to achieve their independent living goals through the provision of SDA services.

Key Responsibilities

  • Client Support
    • Follow up enquiries across multiple regions to ensure timely support for individuals with disabilities.
    • Manage the assessment, planning, and onboarding of residents into SDA properties, ensuring a seamless transition.
  • Expert Advice
    • Provide expert advice regarding SDA, NDIS plans, and budgets to ensure residents are adequately supported.
  • Collaboration and Liaison
    • Liaise with Allied Health professionals, Support Coordinators, and other stakeholders to plan and transition residents into their new homes.
    • Organise property modifications, assistive technology, or other support requirements prior to entry, in consultation with the property team.
  • Onboarding and Administration
    • Undertake the onboarding process in a timely, efficient, and respectful manner.
    • Complete administration tasks associated with the onboarding process.
  • Continuous Improvement
    • Continuously improve service delivery through feedback and evaluation.

Requirements

  • Qualifications
    • Relevant tertiary qualification in disability services, social work, or a related field.
  • Experience
    • Extensive knowledge of SDA.
    • Extensive experience working with people with disabilities, ideally in an SDA or Supported Independent Living setting.
  • Skills
    • Strong communication and interpersonal skills, with the ability to build trust and rapport with clients and external stakeholders.
    • Excellent organisational and time management skills, with the ability to prioritise competing demands.
    • Commitment to the principles of choice, control, and independence for people with disability.
  • Travel
    • Ability to travel interstate as required.
