**Job Opportunity:** Community Transition Support Specialist

We are seeking a skilled clinician to support individuals with severe mental illness transition from custody to the community.

The ideal candidate will have experience working with individuals who experience clinical, psychosocial, and behavioural vulnerabilities, as well as social and emotional wellbeing needs.

The successful applicant will utilise general case management and discipline-specific skills to link patients to relevant services in the community and support their initial reintegration to community care.

Key Responsibilities include:

  • Providing general case management and discipline-specific skills to support patients' transitions back into the community
  • Linking patients to relevant services in the community and supporting their initial reintegration to community care
  • Working collaboratively with multidisciplinary teams to ensure continuity of care

**Required Skills and Qualifications:**

  • Current unrestricted Authority to Practice as a Registered Nurse with the Nursing and Midwifery Board of Australia (for nursing staff)
  • Bachelor's or postgraduate degree in occupational therapy and general registration with Occupational Therapy Board of Australia under Australian Health Practitioner Regulation Agency (Ahpra) (for occupational therapists)
  • Bachelor's or postgraduate degree in social work and professional membership or eligibility for full membership with the Australian Association of Social Workers (AASW) (for social workers)
  • Minimum of a four-year accredited psychology degree and general registration with Psychology Board of Australia under Australian Health Practitioner Regulation Agency (Ahpra) (for psychologists)
