The Community Transitions Clinician will be part of a multidisciplinary team that works in partnership with the Custodial Mental Health Service, primary health networks, Community Mental Health Services and other government and non-government organisations (NGOs) to provide a collaborative approach to ensuring continuity of care. By understanding patients' clinical, psychosocial and behavioural vulnerabilities, and social and emotional wellbeing needs, the Community Transitions Clinician will utilise general case management and discipline specific skills to link patients to relevant services in the community and support their initial reintegration to community care.

The Community Transitions Team is one of the three teams that comprise the NSW Community Forensic Mental Health Services. The team is community based; but provides assertive prison in-reach services to support peoples' transitions back into the community. Its aim is to develop mental health care plans, establish necessary therapeutic supports for patients, and provide follow-up community care to patients leaving custody.
