Job Title: Community Transitions Clinician

We are seeking a highly skilled and experienced Community Transitions Clinician to join our team. The successful candidate will be responsible for supporting individuals with severe mental illness to access the necessary care and support required to ensure a successful transition from custody to the community.

  • The ideal candidate will have a strong background in mental health, with a focus on transitional care.
  • Demonstrated experience in working with multidisciplinary teams is essential.

In this role, you will work closely with various stakeholders, including custodial mental health services, primary health networks, and community mental health services. Your expertise will enable us to provide seamless continuity of care for individuals transitioning back into the community.

Key Requirements:
  • Current unrestricted Authority to Practice as a Registered Nurse or equivalent qualification and at least 3 years' experience working in mental health or a combination of equivalent experience and qualifications.
  • Bachelor's or postgraduate degree in occupational therapy, social work, or psychology, with professional membership or eligibility for full membership with the relevant board or association.
  • Minimum of 3 years' clinical experience as an Occupational Therapist, Social Worker, or Psychologist, with demonstrated extensive specialist knowledge or high level of broad generalist knowledge within the discipline.
