Job Opportunity

We are seeking a skilled and dedicated professional to join our team as a Case Manager in Hervey Bay, QLD. This is a full-time, on-site role that requires strong assessment and care planning skills, excellent communication and interpersonal skills, and experience in coordinating service delivery.

The successful candidate will be responsible for assessing client needs, developing personalized care plans, coordinating service delivery, and providing ongoing support to clients. Key duties include liaison with healthcare professionals, managing client documentation, and ensuring compliance with relevant regulations and standards.

  • Conduct comprehensive assessments of client needs to identify strengths and areas for improvement.
  • Develop and implement individualized care plans that meet the unique needs of each client.
  • Coordinate and monitor service delivery to ensure seamless transitions and high-quality care.
  • Liaise with healthcare professionals to ensure effective communication and collaboration.
  • Manage client documentation and records accurately to maintain confidentiality and adherence to regulatory requirements.
  • Ensure adherence to regulatory and industry standards to protect the rights and interests of clients.
Requirements

  • Experience in coordinating service delivery to ensure seamless transitions and high-quality care.
  • Knowledge of relevant regulations and standards to maintain confidentiality and adhere to regulatory requirements.
  • Organizational and time management skills to prioritize tasks and manage competing demands.
  • Ability to work independently and as part of a team to achieve shared goals.
Preferred Qualifications

While not essential, the following qualifications would be highly regarded:

  • Background in health or social sciences to inform practice and decision-making.
  • Experience working in aged care or community services to understand the sector and its challenges.
Working Conditions

This is a full-time, on-site role based in Hervey Bay, QLD. The successful candidate will be required to work in a fast-paced environment and may be required to travel to meetings and training sessions.
