About the Role

Mission Australia is seeking a skilled Case Manager to join our Youth Accommodation Support Services (YASS) team in Victoria Park. As a Case Manager, you will provide case coordination and support to young people who are experiencing or at risk of homelessness.

Key Responsibilities
  • Complete AOD assessments and intake processes for allocated young people
  • Support young people to create holistic and strength-based individualised AOD support plans
  • Work collaboratively with service staff to provide specialised support to address complex cases or emergency situations
  • Provide brief intervention and informal counselling based on motivational interviewing techniques
  • Facilitate therapeutic and educational groups, covering AOD, mental health and wellbeing, with young people engaged in the service
Requirements for Success
  • A Degree or Diploma in Social Welfare/Social Science/Youth Work or a minimum of three years' experience in a similar role
  • Demonstrated experience providing outreach, case management, case coordination, assessment, counselling and crisis intervention skills
  • Experience and ability to work with at-risk young people, including knowledge of child and adolescent development and child and youth safe practices
  • Well-developed communication and interpersonal skills, including report writing and data collection
