Job Description:

We are seeking experienced Support Coordinators to join our dedicated suicide aftercare program, Care Connect. This exciting opportunity allows you to empower individuals and their caregivers to connect with formal and practical support for recovery.

Your role will involve providing therapeutic supports and short-term psychological interventions, connecting participants to services and supports. You will also have the opportunity to expand your case management and care coordination skills within a multidisciplinary team.

Required Skills and Qualifications:

To be successful in this role, you will need to possess:

  • Diploma or Cert IV level qualification in Mental Health or Community Services
  • At least two years' experience in a suicide prevention / aftercare or mental health setting
  • Previous experience in providing case management to vulnerable groups
  • Demonstrated experience in maintaining professional boundaries while engaging in person-centred work
